    Authorization for Release of Digital Student Records

  • This is an official request for student's educational records. The information contained in this request should be considered private. Please complete all fields below then finalize by clicking "Submit".

    In accordance with the Family Educational Rights and Privacy Act of 1974 and California State Law, we shall only release the requested student's records to the school  named on this form.

    Be advised that our school only contains and provides general education digital records. No physical cume will be mailed.

I understand the recipient of the record/s will use said document/s for legitimate interests only and the information contained therein shall not be further transferred or communicated to any other party or agency without the expressed, written consent of the current/former student except under the authority of Public Law 93-380, Educational Rights and Privacy Act.
